Arthroscopic ACL Reconstruction

 
  The concept of a Major Orthopaedics, Accident Trauma and Neurocentre was inititated by Dr. Thomas A Chandy the Medical Director and Chief of Orthopaedics. He did his MBBS at St. John's Medical College, Bangalore and moved to the US in 1976 for specialization in orthopaedics.
 
   

He did his General Surgery residency at the Bronx - Lebanon Hospital - Albert Einstein Medical School and then 4 years Post Graduation Resident training in 1980 in Orthopaedics at the Jewish Hospital and Medical Centre in New York , he was on the faculty at the same hospital for two years. He is a U.S. Board Certified Orthopaedic Surgeon.

Dr. Chandy did a fellowship at the University of Oklahoma in Arthroscopy, Sports Medicine and knee surgery under Dr. Bill Grana and Don O'Donoher - completed in 1983. He was appointed as the Chief of Orthopaedics at the Claremore Regional Medical Centre, in 1983 and worked there till 1993. At this time, the idea evolved to start a world class Orthopaedics, Sports Medicine Neuro and Accident hospital on an international standard. He is a U.S. Board Certified Orhopaedic Surgeon.

What is Anterior Cruciate Ligament?
 
  The anterior cruciate ligament (ACL) is the major stabilizing ligament of the knee. The ACL is located in the center of the knee joint and runs from the femur (thigh bone) to the tibia (shin bone), through the center of the knee. In this position, it functions to prevent a buckling type of instability of the knee. It is important for daily functions activity and sports.
 
   
How does ACL Injury Occur?  

Usually the tearing of the ACL occurs with a sudden direction change or when a deceleration force crosses the knee. The patient often feels or hears a popping sensation, has the rapid onset of swelling, and develops a buckling sensation in the knee when attempting to change direction.

 
   
Symptoms  
Initially, the symptoms are pain and fairly immediate swelling. After the pain and swelling subsides, patients complain of instability or a feeling of insecurity. The knee feels loose. Some people live in fear of their knee giving out because they are unable to predict the motions that cause the instability.  
   
Diagnosis & Treatment  

Orthopaedic Surgeons may conduct physical tests and take X-rays or an MRI to determine the extent of damage to your ACL. Most of the time, you need reconstruction surgery. HOSMAT is the only hospital to have a KT 1000 orthro meter in India. This instrument helps in the diagnosis of the ACL Tear. The doctor replaces the damaged ACL with strong, healthy tissue. A strip of tendon from under your knee cap (patellar tendon) or hamstring may be used. The doctor threads the tissue through the inside of your knee joint with the arthroscopic method, without opening the knees and secures the end to your thighbone and shinbone.

In a few cases the ACL is torn cleanly from the bone and it can be repaired. People over 50 years of age and People with less activity my be treated non surgically with good physiotherapy and muscle strenghthening exercises.

Arthroscopy Surgery  
Arthroscopy is an endoscopic surgical procedure in which a pencil-sized instrument called an arthroscope is inserted into a joint without any open cut or incision. Fluid is then inserted into the joint to allow the surgeon to thoroughly examine the interior of your knee and determine the source of your problem. During the procedure, the surgeon also can insert surgical instruments through other small pin holes in your knee to repair damaged tissues and cartilage or remove loose peices.  
   
 

Arthroscopy is one of the most common orthopaedic procedures. During the procedure, which is performed under epidural anaeshtesia, with the patient awake, the inside of the joint is examined for damaged tissue. The commonest types of arthroscopic surgery include trimming or repair of a torn meniscus (cartilage), ligament reconstruction, removal of loose debris, and trimming damaged surface cartilage.

Symptoms are swelling, persistent pain, catching, giving-way, buckling and loss of confidence in your knee. Arthroscopies are performed on patients between the ages of 18 and 60.

Arthroscopy is much less traumatic to the muscles, ligaments, and tissues than surgically opening the knee with long incisions. The benefits of arthroscopy are faster healing, quicker recovery and smaller incision.

 
   

A Orthopaedic Surgeon might use this procedure to diagnose or treat:

  • A torn meniscal cartilage
  • Loose fragments of bone or cartilage
  • Damaged joint surfaces or softening of the articular cartilage known as chondromalacia
  • Inflammation of the synovial membrane, such as rheumatoid or gouty arthritis.
  • Abnormal alignment or instability of the knee cap
  • Torn ligaments including the anterior and posterior cruciate ligaments.

It is usually possible to go back to physical activities or sports after around three weeks. The orthopaedic surgeon will provide more advice about this in each individual case.
